- Take time to prepare for your sessions. That will make them more productive. Think of questions and concerns you'd like to bring up; fill out your portion of Joint Patient-Therapist Clinical Forms.
- Arrive a few minutes before your scheduled appointment, sit back and reflect on your progress, goals and issues at hand.
- Bring a list of all medications you take: prescription, over-the-counter, and dietary supplements such as vitamins and herbs.
- Provide us with names and phone numbers of doctors you would like us to communicate with.
- Share with us your questions, concerns and suggestions about improving our services to you.
- Tell us if you have not been compliant with your treatment, including medication.
- Let us know if you are unable to afford your prescribed medication – we may be able to assist you.
- Keep your appointments as scheduled; if you must cancel give us at least 24 hours’ notice to allow us to allot that time to someone else.
- Mention at the beginning of your visit if you need us to fill out forms for you.
- Know your insurance benefits, including co-pays and deductibles, let us know when your insurance changes.
- Generally, payment is expected at the time of service. We will try to be flexible if you need to make other arrangements. We accept checks, cash, Visa and MasterCard.
- We will make every effort to collect from your insurance company; any unpaid balance is your responsibility.
